Welcome. The questionnaire on this site is designed to help you and your counselor assess your concerns at the beginning of counseling, and to keep track of changes as time goes on. Our commitment is to provide you with the highest quality counseling experience possible.

FOLLOW THESE STEPS TO COMPLETE THE QUESTIONNAIRE:

1) Click “Student Login” on the right side of this screen

2) Enter “User ID” and “Password” (obtained from our office) and click “Login”

3) Read directions and click on “Click here to begin your Assessment”

4) Scroll down to Outcome Questionnaire, answer questions and select “Document Complete”

If you have not yet been provided a User ID and Password, please contact our reception staff at 232-2468 for assistance. You may also contact that number with any other questions.

*Security note: This internet system is fully HIPPA compliant. Through the use of secure portals the system allows individuals to take assessments and providers to receive results without any personal data passing over the web.
